Debugger fails to launch, no reason given?

i'm on a mac.

i occasionally come across this when i run:

no reason given, it just doesn't run. i see in the lower right:

LOOKS like launching the debugger failed.
but Qt Creator doesn't provide ANY information about what went wrong. where can i look?

Window > Views > Debugger Log
