Standard button Exit won't work, how to fix?

Probably a silly question: my Exit button in application (run in Qt simulator) won't work anymore, neither other button, for which I set up action/signal manually. I must have been set something wrong but can't find what exactly. No relevant signals been messed with in constructor etc. Appreciate any suggestions about how to fix it.

Hi,
In such kind of situation source code is mandatory :) but it will be also nice to state which version of Qt are you using and for which OS are you building it.
