Qt Designer Form Class

I click New->New File->Qt->Qt Designer Form Class. A window with some form templates open. How can i add my form template to that window?

no i dont want custom widget. i want to make ui form and add it into qt designer form class wizard

hi, i think it is not,
because i don't want custom wizard, i want to add ui file template to qt->qt designer form class wizard
