I have the same problem here, but I am running Qt 5.12.11 on an Intel Mac. The Errors are random and always I get a stack trace which is identical to KejPi below (None, PNGReadPlugin::InitializePluginData ...). I installed Qt with the Qt Installer and the same app runs on Windows without any problems.
Today I did an update to Monterey 12.5 but the random error still appears .....
