How to create UI Forms qml project ?

I can not find how to create UI Forms project..
I do : File > New file or project > Applicaton > then there is no more UI Forms projet
My Qt Creator version is 4.7.2

I have same as you, except for Python things, but even in your screen i can't see the "UI Forms Application"
I don't want 'normal' qt quick app, i wanted to test the approch where you have files like MyButton.ui.qml

edit : In "Other Project" tab there is an entry called "Qt Quick UI Prototype" but it says it is only for prototyping

if i do

I get project with qml.ui file but if thats the same as the other project option, i dont know :)
