How to get popup window with checkbox on button click

Hi ,

Can anyone help me how can I get a set of check boxes in a popup window on button click . where in I can choose multiple check boxes and on checking those boxes respective form should get open.

I have used Qmenu with QAction Widget to get my requirements done.
