I was applied layout before adjusting size. Why after I clicked the Ajust Size button did the form shrink to nothing?

Because you don't have a layout on the widget itself.
Is there one on the Primary Key groupbox?
Right click into the groupbox and add a vertical box layout (if that is missing).
Then right click on the background of the widget itself and add a horizontal box layout.
