How to create a form that stores the values in a Database?

Hi,

I want to create a form which gets the value from the user and stores it in a database. Since I'm new to -QT- Qt I need some guidance in form creation and integrating database with -QT- Qt creator. Please post back some links which consists of step by step instructions for creating a form linked with database.
