State in contact form should be hidden when not relevant

The State combobox in the contact form (reached through the trainings page) shows the text "Select a value" for most countries I tried and correctly shows states when the USA is tried.

It should maybe be hidden for country != USA.

Also in the contact form, the Navigation box on the right allows clicking on Country or State, offering a subset of the full form that can be submitted.

It's not clear what happens when that form is submitted, but the http://qt.nokia.com/services-partners/training/contact-form/Country should probably not be available. I imagine the web framework does most of the splitting there automagically.

That form is controlled by the CRM system, it's not a lot I can do with it, sorry.
