Looks like simple UI, you can use designer to create it or do it manually with code.
You can create it with designer and then inspect generated code.
Use layouts to arrange widgets and ask more specific questions if it doesn't work as expected... -

It's already available within Qt Creator. If you want to use the standalone application, it's provided in the bin folder of the Qt version(s) you installed.
