QT4: is it possible for focus out not to happen

My client reported bahaviour, that would basically mean that focus out event is not happening when he changes focus (via mouse click) to another control. I could not reproduce this behaviour. We do not filter focus out events in any way. I do not really see how it is possible, is it?
