Where is the Wizard for "Qt Designer Form Class" located?

I'm trying to replicate the exact same Wizard, but i can't find said wizard in the QTCreators wizard folder. Can anyone point me to the correct location? I can find any other Wizard for single files, classes etc. but not the designer form class comprised of header, implementation AND UI file.
