I couldn't figure out the issue

Which Module am i missing out for this specific lib to not load ??

In principle I would have expected this to come with the ActiveQtC component which you have already checked. One thing: it's trying to link with a Debug version of the library, are you indeed compiling for Debug or for Release? There is one web item about possible issues with Qt going for the Debug version even if compiling for Release. What Build system do you use? And I assume you are indeed building for Qt5.

I'm afraid you need someone else to answer now. I don't use VS (in fact I am a Linux user) or know what your issue stems from. You might verify that wherever you have Qt libraries (check for both
.libs &.dlls) you have the otherQt5Ax....lib/dllones but notQt5AxBasefollowed by anything. You might also set up to create a small project in Qt Creator to confirm it goes wrong there too. -
