Where is "Qt GUI Application"?!!!

Sorry guys if its a stupid question
I just downloaded Qt Creator 5.3.1 for Windows but i can't find "Qt GUI Application" and i searched for Qt SDK but i can't find it too.Thanks

Look at your c:\ for Qt paste or try to search for Qt Creator.

Qt Creator is just an IDE. Alone it's basically a feature-rich text editor ;).
It doesn't contain Qt library or a compiler. You need to download these separately. Qt Creator will then pick it up and enable appropriate wizards (like the Qt GUI application).
Also 5.3.1 is not a version of Qt Creator. It's a version of Qt library. Current version of the creator is 3.something.something.
The whole thing is easier if you just use an online installer to install your components - IDE, Qt library and compiler (in case of MinGw). Alternatively you can install one of the bundles (IDE+library). Head to the "download page":http://qt-project.org/downloads for one of these. -
I know where is Qt Creator and i already created some C++ projects

Well that's because there isn't one. There are 2 types of UI in Qt. Widgets or QML and that's what you see there: "Qt Widgets Application" and "Qt Quick Application".
Where did you get the name "Qt GUI Creator"? -
Well i know its IDE and sorry about the version i just checked, mine is 3.1.2
i downloaded the online installer but i didn't see GUI library, all i saw was compilers and i download MinGw 4.8.2
i want to download the GUI library, how can i download and add it to Qt creator?

The screenshot you posted says you already have it. Qt Widgets and Qt Quick are both UI applications.

Ahh thank you but why the youtube tutorial create "Qt GUI Application"?
and the wiki here say create Qt GUI Application

The pictures in that link are dated 2011. A lot has changed in the past 3 years. Both Qt and Qt Creator went through significant changes over the years.
Back then QtQuick and QML were very fresh and "experimental". As you can see there was a clear distinction to C++ apps and Qt Quick apps (the left panel). Later on they were merged to a single "Applications" entry and given names indicating technology involved. -

The wiki tutorials, like the one you found, are a community effort and are not really maintained unless the user that created it or someone else is interested enough, which doesn't happen often.The "official documentation":http://qt-project.org/doc/ on the other hand is part of every Qt release and is(mostly) kept fresh and up to date. Qt Creator and the Qt library have their separate documentation and examples.
"Here":http://qt-project.org/doc/qtcreator-3.1/creator-writing-program.html you will find a very basic widget app example for the current version of Qt Creator.
There are many others in the "getting started":http://qt-project.org/doc/qt-5/gettingstarted.html section. There are also many examples ranging from basics to more advanced topics distributed alongside Qt libraries. Look through examples and tutorials by pressing the corresponding buttons on the Qt Creator welcome page. -
